MBBS Final Year: Subjects, PYQs and Syllabus

MBBS Final Year is Phase 3 Part 2 of the NMC Competency-Based Medical Education (CBME) curriculum, covering four major clinical subjects: General Medicine, General Surgery, Obstetrics and Gynaecology (OBG), and Paediatrics. This is the most clinically intensive phase of the MBBS — all four subjects involve long inpatient and outpatient postings, bedside clinical examinations, and theory papers that are substantially case-framed. GeckoMed provides previous year question papers, phase-wise syllabus, important questions, viva prep, and clinical topic pages for all four subjects across all seven target universities.

Final Year MBBS Subjects

Phase 3 Part 2 has four paper-bearing subjects, each with both theory and clinical examination components. The bedside or OSCE examination — in which students assess a real patient in front of examiners — carries significant weight alongside the theory paper in all four subjects.

General Medicine

General Medicine is the broadest clinical subject in the MBBS, covering cardiovascular disease, respiratory disease, gastroenterology, nephrology, neurology, haematology, endocrinology, infectious diseases, and rheumatology. It is also the subject with the largest and most varied question bank across university papers. The important questions pages for General Medicine are the most efficient starting point for revision: they distil the highest-frequency topics from PYQ analysis across all confirmed years.

General Surgery

General Surgery covers surgical anatomy, wound healing, fluid and electrolyte management, abdominal surgery, breast and thyroid surgery, vascular surgery, orthopaedics (fractures and joint conditions), and trauma principles. Surgery theory papers draw from both the clinical and anatomical knowledge base, and surgical anatomy questions — which integrate Phase 1 Anatomy with clinical surgical context — are a consistently tested category across all seven universities.

Obstetrics and Gynaecology

Obstetrics and Gynaecology covers the full spectrum of women's reproductive health: antenatal care, labour and delivery, obstetric complications, postpartum care, gynaecological conditions, and reproductive endocrinology. OBG is a subject that combines surgical skill (operative obstetrics) with clinical medicine (medical complications of pregnancy) and requires students to be comfortable with both dimensions in the bedside examination.

Resources:

Paediatrics

Paediatrics covers the health and disease of children from the neonatal period through adolescence, including neonatal care, growth and development, vaccination schedules, common paediatric infections, nutritional disorders, and paediatric emergencies. Paediatrics is often underestimated in terms of syllabus breadth. The neonatal section in particular — covering neonatal jaundice, respiratory distress, sepsis, and birth asphyxia — is heavily tested across all university papers and requires dedicated preparation separate from the general paediatric content.

Final Year MBBS Syllabus 2026

MBBS Final Year follows the NMC CBME syllabus for Phase 3 Part 2. The CBME framework for this phase is the most clinically integrated in the MBBS: competencies are defined around patient assessment, clinical reasoning, and management, not purely around factual recall. Theory papers reflect this — long essay questions in Medicine, Surgery, OBG, and Paediatrics are routinely case-framed, and short notes frequently test applied clinical judgement alongside factual knowledge.

MBBS Final Year Question Papers (2025 to 2021)

Tier-1 universities (RGUHS, KNRUHS, TNMGRMU) have five paper years (2025, 2024, 2023, 2022, 2021). Non-Tier-1 universities (NTRUHS, WBUHS, KUHS, RUHS) have three paper years (2025, 2024, 2023).

How to Study for MBBS Final Year

  • Use important questions pages as your General Medicine entry point, not the textbook. General Medicine covers more conditions than any other MBBS subject, and open-ended textbook reading without PYQ guidance produces uneven preparation. The important questions pages compile the highest-frequency topics across confirmed paper years. Start there, read the relevant clinical topic pages, then use PYQs to practise your answers.
  • Prepare the bedside examination as a separate, active track. The clinical component of Final Year exams is assessed in front of examiners with a real patient. Theory revision alone does not prepare you for this. Use posting time to take histories, present cases, and practise systematic examination of each organ system. The gap between knowing the content and demonstrating it at the bedside is where most Final Year marks are lost.
  • Treat Surgery and Anatomy as integrated, not separate. Final Year Surgery papers consistently test surgical anatomy — the applied version of Phase 1 content. Reviewing the anatomy of structures involved in high-frequency surgical conditions (hernias, appendix, thyroid, biliary tree, breast, major vessels) improves both theory answers and clinical examination performance.
  • Give the neonatal section of Paediatrics its own preparation time. Neonatal jaundice, neonatal sepsis, respiratory distress syndrome, and birth asphyxia are disproportionately represented in Paediatrics papers across all seven universities. Students who treat Paediatrics as a single undifferentiated syllabus often underinvest in neonatal content and lose marks in a predictable question category.
  • OBG requires preparation in both obstetric and gynaecological sections independently. The two sections draw from different clinical knowledge bases and are often examined with separate question sets within the same paper. Prepare them as two parallel tracks rather than as one subject, and prioritise complications of pregnancy and gynaecological emergencies from PYQ analysis.

Frequently Asked Questions

How many subjects are there in MBBS Final Year?

MBBS Final Year (Phase 3 Part 2) has four paper-bearing subjects under the NMC CBME curriculum: General Medicine, General Surgery, Obstetrics and Gynaecology, and Paediatrics. Each has a theory examination and a clinical bedside assessment conducted by university examiners.

Which is the most difficult subject in MBBS Final Year?

General Medicine is considered the most demanding Final Year subject by breadth — it covers more organ systems and conditions than any other MBBS subject, and the theory paper reflects that range. General Surgery is considered demanding by integration, requiring both clinical and anatomical knowledge. OBG and Paediatrics each require preparation across two distinct sub-domains. Most students find the volume manageable with systematic PYQ-guided preparation rather than open-ended reading.

Are Final Year MBBS question papers different across universities?

The NMC CBME syllabus defines the subject content uniformly for all affiliated colleges, but each university's papers carry its own examiner preferences and emphasis patterns. High-frequency topics like Diabetes Mellitus and Tuberculosis in Medicine, or appendicitis and hernias in Surgery, appear across all universities — but the question framing and the specific aspects tested vary. University-specific PYQ archives on GeckoMed are the most reliable guide to what your examiner prioritises.
