MBBS 2nd Year Syllabus 2026 (Subject-wise)

The second year of MBBS corresponds to Phase 2 of the National Medical Commission (NMC) Competency-Based Medical Education (CBME) curriculum. It introduces four core subjects: Pathology, Pharmacology, Microbiology, and Forensic Medicine. This is a pivotal year: while Phase 1 builds the structural and functional sciences (Anatomy, Physiology, Biochemistry), Phase 2 introduces disease mechanisms, drug therapy, and the science of infection -- all of which form the foundation for clinical reasoning in Final Year. This page gives you a subject-wise syllabus overview, exam pattern, recommended textbooks, and direct links to question papers for all seven GeckoMed target universities. Content is aligned with the NMC CBME framework and updated for the 2026-27 academic year.

Subject-wise Syllabus Overview

Pathology Syllabus (MBBS 2nd Year)

Pathology in Phase 2 covers General Pathology, which forms the conceptual foundation for understanding disease across all clinical subjects. Topics include:

Cell Injury and Cell Death: Causes of cell injury, reversible and irreversible changes, necrosis vs apoptosis

  • Inflammation and Repair: Acute and chronic inflammation, granulomatous inflammation, wound healing
  • Circulatory Disturbances: Oedema, congestion, thrombosis, embolism, infarction, haemorrhage, shock
  • Neoplasia: Nomenclature, carcinogenesis, tumour markers, differences between benign and malignant tumours
  • Immunopathology: Hypersensitivity reactions (Types I to IV), autoimmune disease, immunodeficiency
  • Genetic and Paediatric Diseases: Chromosomal disorders, single-gene disorders
  • Haematology: Anaemia classification, leukaemias, lymphomas, bleeding disorders

Many universities also introduce selected Systemic Pathology chapters (such as cardiovascular and respiratory pathology) in the 2nd Year practical syllabus. The practical component covers haematology, histopathology slide identification, and basic laboratory techniques.

Pharmacology Syllabus (MBBS 2nd Year)

Pharmacology is one of the most high-yield subjects in the 2nd Year, with topics reappearing frequently in Final Year clinical papers. The NMC CBME curriculum covers:

  • General Pharmacology: Pharmacokinetics (absorption, distribution, metabolism, excretion) and pharmacodynamics (receptor theory, dose-response relationships, drug interactions)
  • Autonomic Nervous System Pharmacology: Cholinergic, adrenergic, and related agents
  • Central Nervous System Pharmacology: Sedatives, hypnotics, antiepileptics, antipsychotics, antidepressants, opioids
  • Cardiovascular Pharmacology: Antihypertensives, antiarrhythmics, antianginal agents, diuretics, anticoagulants
  • Chemotherapy: Antibiotics (classification and mechanism), antifungals, antivirals, antiprotozoals, anthelmintics
  • Autacoids and Related Agents: Histamine, NSAIDs, corticosteroids
  • Endocrine Pharmacology: Insulin, oral hypoglycaemics, thyroid agents, sex hormones
  • Gastrointestinal Pharmacology: Antacids, antiemetics, laxatives

The practical component covers experimental pharmacology, prescription writing, and drug identification exercises.

Microbiology Syllabus (MBBS 2nd Year)

Microbiology in Phase 2 covers the science of micro-organisms in relation to human disease. The NMC CBME curriculum includes:

  • General Microbiology: Bacteriology basics, sterilisation and disinfection, culture media and techniques, normal flora
  • Immunology: Innate and adaptive immunity, antigens, antibodies, hypersensitivity, vaccines
  • Systemic Bacteriology: Gram-positive and Gram-negative organisms of clinical significance, mycobacteria (including Mycobacterium tuberculosis and Mycobacterium leprae)
  • Virology: DNA and RNA viruses of clinical importance, HIV, hepatitis viruses
  • Mycology: Superficial and systemic fungal infections
  • Parasitology: Protozoa, helminths, and arthropods of clinical significance
  • Clinical Microbiology: Laboratory diagnosis, antibiotic sensitivity testing, specimen collection

Practical sessions include culture and sensitivity, staining techniques (Gram stain, Ziehl-Neelsen stain), and slide identification.

Forensic Medicine Syllabus (MBBS 2nd Year)

Forensic Medicine and Toxicology is unique among Phase 2 subjects in its legal and medico-legal orientation. The NMC CBME curriculum covers:

  • Medical Jurisprudence: Indian Medical Council Act, consent, medical negligence, professional ethics, rights and duties of a medical officer
  • Forensic Pathology: Manner and cause of death, postmortem changes, autopsy procedures
  • Thanatology: Signs and changes after death, time since death estimation
  • Forensic Traumatology: Classification of wounds, injuries, firearms, road traffic accidents, sexual offences
  • Toxicology: General principles of poisoning management, specific poisons (corrosives, irritants, CNS poisons, cardiac poisons, miscellaneous agents)
  • Clinical Forensic Medicine: Examination of accused, victims, and injury certification

The clinical component involves case presentations and medico-legal documentation exercises.

Exam Pattern: MBBS 2nd Year

The Phase 2 university examination follows the NMC CBME assessment framework. The general structure applies across all affiliated universities, though specific marks and paper formats are notified by each university at the time of examination.

Theory Paper Format Each subject carries a theory examination comprising:

  • Multiple Choice Questions (MCQs) in a computer-based or paper-based format, depending on the university
  • Short Answer Questions (SAQs) requiring concise structured responses
  • Long Answer Questions (LAQs) requiring detailed descriptive answers

Practical and Clinical Examination All four Phase 2 subjects include a practical examination. Pathology, Pharmacology, and Microbiology include laboratory practicals and viva. Forensic Medicine includes a clinical and viva component involving medico-legal case discussion.

Internal Assessment Continuous internal assessment is conducted throughout the academic year and contributes to the final result as per NMC CBME regulations. This typically includes term tests, practical records, seminar presentations, and attendance. A minimum attendance requirement applies across all subjects.

Preparation Strategy The most effective approach to Phase 2 exams is to integrate subjects: many Pharmacology questions are answered better when Pathology mechanisms are understood, and Microbiology organism profiles align with Pharmacology antibiotic chapters. Reviewing previous year question papers by subject is the fastest way to identify high-frequency topics.

Frequently Asked Questions

What are the subjects in MBBS 2nd Year?

MBBS 2nd Year (Phase 2 under NMC CBME) has four subjects: Pathology, Pharmacology, Microbiology, and Forensic Medicine. All four are examined with theory and practical components at the end of the academic year.

Is the MBBS 2nd Year syllabus the same across all universities in India?

The core syllabus follows the NMC CBME competency framework, which is common to all Medical Council of India-recognised colleges in India. However, individual affiliated universities may vary in their paper format, internal assessment structure, and which topics they emphasise in exams. Use GeckoMed to access university-specific question papers and track recurring topics.

Which subject is hardest in MBBS 2nd Year?

Pharmacology and Pathology are most commonly cited by students as the highest-volume subjects due to the breadth of their content. Pharmacology has particular importance because its topics reappear throughout Final Year clinical papers. Microbiology and Forensic Medicine, while more self-contained, carry dense factual content that benefits from regular revision.
