It is the single national exam that decides MBBS admission across the country, replacing every separate state and college-level entrance test that used to exist. What Is NEET UG?
NEET UG is conducted by the National Testing Agency (NTA) and is the sole entrance exam for MBBS, BDS, and other undergraduate medical and allied health courses in recognised Indian institutions. Every seat, government or private, in every state, is filled through this one exam and the centralised or state-level counselling process that follows it. There is no separate college-specific entrance test anymore; your NEET UG score and rank are what colleges use to offer you a seat.

There are no optional questions or internal choices in the current format. Every question must be attempted or left blank; there is no partial-choice section where you pick a subset of questions to answer.
The Type of Questions NEET UG Asks
NEET UG questions are multiple-choice, based on the Class 11 and 12 NCERT syllabus in Physics, Chemistry, and Biology. Biology carries the highest weightage of the three subjects, split evenly between Botany and Zoology, which reflects how central biological sciences are to a career in medicine. Physics and Chemistry questions typically test applied problem-solving and conceptual understanding rather than pure recall, while Biology questions lean more on precise factual knowledge, diagram-based recognition, and process-based reasoning.
Because of the negative marking scheme, accuracy matters more than the number of questions attempted. Guessing on unfamiliar questions can cost you more than it gains, which is why NEET preparation typically emphasises strong fundamentals over speed alone.

That balance, more than any single question, is what NEET UG actually tests across its full 180-question paper.
How Your NEET Score Determines Your MBBS Seat
Once NEET UG results are declared, your score and All India Rank feed into the counselling process, which is run partly at the national level (for a share of seats across states, and for central institutions) and partly by individual state counselling authorities for state quota seats. You fill in your college preferences during counselling, and seats are allotted round by round based on your rank, your category, your domicile state where relevant, and the preferences you have listed. The exam itself only produces the score and rank; the counselling process is what actually places you at a specific medical college.
Why the Subjects Change Completely Once You Join MBBS
NEET UG tests Physics, Chemistry, and Biology at the school level, largely following the NCERT Class 11 and 12 syllabus. Once you join MBBS, the subjects change entirely. 1st Year MBBS begins with Anatomy, Physiology, and Biochemistry, under the NMC Competency Based Medical Education (CBME) curriculum. By 2nd Year you move into Pathology, Pharmacology, Microbiology, and Forensic Medicine. 3rd Year adds Community Medicine, ENT, and Ophthalmology, and Final Year is built around General Medicine, General Surgery, Obstetrics and Gynaecology, and Paediatrics.
None of this overlaps directly with the NEET syllabus, beyond the general scientific foundation Biology gives you. It is a new curriculum, a new exam pattern entirely (university theory papers, practicals, and viva voce assessments instead of a single MCQ paper), and a very different day-to-day rhythm of study built around dissection halls, wards, and clinical postings rather than classroom lectures alone.
A Note for NEET Aspirants
If you are still preparing for NEET UG, know that the transition into MBBS is a genuine reset. The subjects, the exam format, and the study rhythm are all different from what got you here, and that is true for every student, regardless of how they scored in NEET. Getting into MBBS is the first milestone. Once you are in, the actual work of learning medicine starts from a fresh foundation, and every 1st Year student, however they were ranked, begins Anatomy, Physiology, and Biochemistry on the same footing.
